4 SIMPLE STEPS


Obtain a Hockey Canada Injury Claim Form below or from your ASHL League Manager.


Completely fill out both sides of your injury claim form and ensure the ASHL League Manager signs off on your form within 90 days of the injury.


Include all original copies of receipts pertaining or relevant to your claim. If your claim includes Loss of Benefits, please include: T4 Slip; Letter from your Employer; & an Employee Cheque Stub.


Mail your completed Injury Claim to the Address Listed at the Bottom of Page 1 on the Hockey Canada Injury Claim Form

**Please be advised that Claims may take upwards of 6-8 weeks to process

Playing in the United States?


Report the injury to your League Manager in Fort Wayne or Romeoville. The League Manager will subsequently provide you with an injury report form.


Completely fill out the injury report form and return it to your League Manager to sign off on.


Mail your completed injury report to K&K Insurance Group, INC. / Specialty Benefits, INC. Claims Department (P.O Box 2338, Fort Wayne, IN, 46801-2338)


You should subsequently receive a claim number from K&K Insurance that you can use moving forward with your treatment.
